Patient Intake Form

If eligible, you'll receive an email with a link to the New Patient Intake Form. Our nurse can assist you if needed.

Step 3

Free Pre-Screening Call

One of our nurses will call you to discuss the process and schedule your doctor appointment.

Step 4

Doctor Consultation

Attend a 15-minute telehealth consultation with an independent, AHPRA-registered doctor.

Step 5

Prescription Dispensary

Your e-script is sent instantly to Dispense Direct or another pharmacy of your choice.

Step 6

Ongoing Patient Care

Our nurses will check in after your first appointment. Contact our team anytime for support.

What to Expect

During Your Nurse Consultation

Your free nurse consultation is an opportunity to discuss your health needs and determine if our services are right for you. Our friendly nurses will review your medical history, explain the process, and answer any questions you have. This consultation is completely free, and there's no obligation to proceed.

During Your Doctor Consultation

Your 15-minute telehealth consultation is conducted via secure phone. The doctor will review your health history, discuss your symptoms and goals, and determine if alternative medicine treatment is appropriate for you. If treatment is prescribed, an e-script is sent directly to your chosen pharmacy.

After Your Appointment

Our care doesn't stop after your consultation. Our nursing team will check in with you to see how you're going. You can contact us anytime on 1300 421 597 for support. Follow-up consultations are recommended every 3 months minimum to review your treatment plan.
